Preface
\nOur goal with this handbook is to provide a practical, evidence-based resource
\nfor generalists and addiction specialists. Addiction is a common medical prob-
\nlem and one of the leading causes of preventable morbidity and mortality
\nworldwide. We hope that this book will help clinicians feel more comfortable
\nwith\u2014and be more competent in\u2014evaluating and treating their patients with
\nsubstance use disorders.
\nAs primary care physicians who are also involved in addiction treatment,
\nwe have incorporated addiction treatment into our own primary care practice,
\nand this has provided us with a unique perspective. We have also been involved
\nin teaching medical students and house staff about substance use disorders for
\nmany years and have found that trainees\u2019 attitude toward and comfort with
\ntreating individuals with these problems can improve with experience and
\npositive role modeling. We hope that this experience has helped us present this
\nissue in a manner that \u201cmakes sense\u201d for other clinicians. While patients with
\nsubstance use disorders are sometimes thought of as \u201cdifficult,\u201d we have found
\nthat treating these individuals is gratifying and that they are appreciative of
\nclinicians who show interest in them, are knowledgeable about the treatment
\nof their problems, and treat them with respect.
\nThis handbook is an update of our 2015 handbook. In writing this hand-
\nbook, we have had the opportunity to improve upon the previous edition and
\nto update the content. As with the previous edition, we have strived to make
\nit \u201cevidence-based\u201d; by this we mean that the information we present is based
\non primary data, whenever possible. We have conducted an extensive review of
\nthe literature in each of the topic areas we covered. Furthermore, we have done
\nour best to explicitly present our sources and to critically evaluate them. It is for
\nthis reason that we have provided numerous references and have included addi-
