PREFACE
The evaluation of lymph nodes and classification of lymphomas and
nonneoplastic lymphoid proliferations begins with an
understanding of the morphologic clues. In addition to histology,
modern lymph node pathology requires a host of additional
ancillary studies, which have become routine in diagnostic
hematopathology. This multidisciplinary approach to lymph node
pathology has also become the foundation of modern lymphoma
classifications. This has enabled pathologists from different parts of
the world to use standardized approaches to diagnosis. It has also
improved our communication with practicing clinical hematologists
and oncologists, and ultimately contributed to improved treatment
of lymphoid neoplasia.
In this book, we present neoplastic lymph node pathology
according to the 2022 International Consensus Classification (ICC).
However, since both the ICC and the fifth edition of the World
Health Organization (WHO) Classification are likely to be widely
used during this transitional period in our field’s history, we have
included both classifications in the appendix.
Recent advances in hematopathology have been presented in
several voluminous textbooks. In contrast to these comprehensive
compendiums, we present here our own practical diagnostic
approach tested over multiple years in our respective busy academic
hematopathology practices. We hope that hematopathologists, as
well as general surgical pathologists and pathology trainees, will
find this book useful in their sign-out rooms.
In contemporary medical practice the prevailing trend is to use
less invasive methods for obtaining diagnostic materia

Accordingly, the specimens submitted to pathologists are often small
core biopsies. However, as morphologists first, we recognize that no
amount of ancillary testing can replace an adequate specimen for
morphologic review in most instances. This book seeks to provide a
foundation for morphologic assessment of both excisional and core
needle lymph node biopsies and emphasize the challenges of small
specimens. Cognizant of our diagnostic limitations under these
conditions, we advocate a balanced approach and good
communication with clinicians. We realize that sometimes more
tissue may be required for a definitive diagnosis.
Targeted and judicious use of ancillary studies is emphasized
throughout the text. Most notably we focus on the most useful
immunohistochemical panels, which form the backbone of our daily
lymph node evaluations. From numerous cytogenetic and molecular
findings in different entities, we have chosen to highlight more
relevant ones for diagnosis and prognostication, while recognizing
that this is a rapidly evolving field.
Although we strove to include all of the pertinent information in
describing the various neoplastic and nonneoplastic entities
involving lymph nodes, the true focus of the book is morphology
and pattern recognition, the importance of which cannot be
overemphasized in lymph node pathology. We have sought to share
our everyday experience and practice of lymph node pathology in
this text and we hope that you will enjoy reading it.
