These approaches<br \/>\ncan be equally effective, and they are probably<br \/>\nnot as dissimilar as their descriptions would suggest. In reality, even<br \/>\n\u201cGestaltists\u201d subliminally examine details of an image, and if asked spe-<br \/>\ncifically about particular features of it, they are able to say whether one<br \/>\ncharacteristic or another is important diagnostically.<br \/>\nIn accordance with these concepts, in 2004 we published a text-<br \/>\nbook entitled Practical Pulmonary Pathology: A Diagnostic Approach<br \/>\n(PPPDA). That monograph was designed around a pattern-based<br \/>\nmethod, wherein diseases of the lung were divided into six categories<br \/>\non the basis of their general image profiles. Using that technique, one<br \/>\ncan successfully segregate pathologic conditions into diagnostically<br \/>\nand clinically useful groupings.<br \/>\nThe merits of such a procedure have been validated empirically by<br \/>\nthe enthusiastic feedback we have received from users of our book. In<br \/>\naddition, following the old adage that \u201cimitation is the sincerest form<br \/>\nof flattery,\u201d since our book came out other publications and presenta-<br \/>\ntions have appeared in our specialty with the same approach.<br \/>\nAfter publication of the PPPDA text, representatives at Elsevier, most<br \/>\nnotably William Schmitt, were enthusiastic about building a series of<br \/>\ntexts around pattern-based diagnosis in pathology. To this end, we have<br \/>\nrecruited a distinguished group of authors and editors to accomplish<br \/>\nthat task. Because a panoply of patterns is dif-<br \/>\nficult to approach mentally from a practical<br \/>\nperspective, we have asked our contributors to<br \/>\nbe complete and yet to discuss only principal<br \/>\ninterpretative images. Our goal is eventually to<br \/>\nprovide a series of monographs that, in combi-<br \/>\nnation with one another, will allow trainees and<br \/>\npractitioners in pathology to use salient mor-<br \/>\nphologic patterns to reach with confidence final<br \/>\ndiagnoses in all organ systems.<br \/>\nAs stated in the introduction to the PPPDA text, the evaluation of<br \/>\ndominant patterns is aided secondarily by the analysis of cellular com-<br \/>\nposition and other distinctive findings. Therefore, within the context<br \/>\nof each pattern, editors have been asked to use such data to refer the<br \/>\nreader to appropriate specific chapters in their respective texts.<br \/>\nWe have also stated previously that some overlap is expected<br \/>\nbetween pathologic patterns in any given anatomic site; in addition,<br \/>\nspecific disease states may potentially manifest themselves with more<br \/>\nthan one pattern. At first, those facts may seem to militate against the<br \/>\nvalue of pattern-based interpretation; however, pragmatically they do<br \/>\nnot. One often can narrow diagnostic possibilities to a few entities<br \/>\nusing the pattern method, and sometimes a single interpretation will<br \/>\nbe obvious. Both outcomes are useful to clinical physicians caring for<br \/>\na given patient.<br \/>\nIt is hoped that the expertise of our authors and editors, together<br \/>\nwith the high quality of morphologic images they present in this Else-<br \/>\nvier series, will be beneficial to our reader-colleagues.<br \/>\nKevin O.
