This is the fifth edition of Ioachim’s Lymph Node Pathology but it is the first edition without
Harry L. Ioachim, MD, who sadly passed away on June 6, 2021. The vision and format of this
textbook were the brainchild of Dr. Ioachim who had a highly successful, over 50-year career as
a practicing pathologist, researcher, author, and administrator before he retired. As he wrote in
the preface of the first edition, “It appears that a need exists for a comprehensive source of
information on the pathology of lymph nodes. To be of assistance to practicing pathologists who
must evaluate lymph node biopsies, such a book must be inclusive, practically oriented and well
illustrated.” The first edition of this textbook, written solely by Dr. Ioachim, was published in
1982 and was very well received. Dr. Ioachim then followed with a sole author second edition
published in 1995, a third edition coauthored with Howard Ratech, MD, published in 2002, and a
fourth edition coauthored with myself in 2008. When Harry asked me to help with the fourth
edition, he asked me which chapters I wished to write. After I told him he could decide (after all,
it was his book), he copied the table of contents of the third edition and placed a blue or red
checkmark next to each chapter. He wrote in a note, “since I am from a blue state and you are
from a red state, I suggest we divide the writing of chapters according to these checkmarks, if
you agree.” (I did agree and took on all chapters checked in red.) It was an honor to be asked by
Harry to coauthor the fourth edition, and it is my goal and responsibility to try and maintain the
high standards set by Dr. Ioachim in this fifth edition. With each edition, Ioachim’s Lymph Node Pathology has grown in its number of chapters,
pages, and images as our understanding of lymph node diseases has expanded. In part, our
increased knowledge can be attributed to the advent of monoclonal antibodies and the
application of flow cytometric and immunohistochemical methods to immunophenotype lymph
node lesions. Equally revolutionary was the development of cytogenetics and molecular biologic
methods and their application to the study of lymph node lesions, particularly neoplasms. With
the flood of new information, it has been challenging to judiciously revise older chapters as well
as add new chapters to cover recently recognized entities. As Harry advised me during the
preparation of the fourth edition, and as he wrote in the preface of that edition, “Striking a proper
balance is important in producing a book. Exhaustive coverage of every subject is better left to
the large multi-authored tomes; coverage that is too concise is prone to being misunderstood and
misused.” As I prepared the current edition, it is this balance that I have strived to achieve for
